It does not replace MoodClinics’ Notice of Privacy Practices, which governs protected health information created or maintained as part of clinical care. The public getting-started questionnaire is anonymous. Answers remain in your browser for the current session and are not transmitted to, stored by, or reviewed by MoodClinics.
If you email MoodClinics, we receive the information you include, such as your email address and message. Please do not send diagnoses, medications, symptoms, identification documents, or other sensitive medical information by ordinary email.
Our web host may automatically process limited technical information required to deliver and protect the site, such as IP address, browser type, device type, requested pages, dates and times, and security logs. MoodClinics does not currently use advertising trackers or sell personal information collected through this website.
3. Patient portal and clinical information
When a secure patient portal is made available, registration, intake, messages, appointments, and clinical information should be completed there rather than on this public website. Information submitted through a third-party patient portal is also governed by that provider’s privacy and security terms and by applicable healthcare privacy law.

You may request access, correction, or deletion of website inquiry information by contacting us, subject to identity verification and legal or healthcare record-retention requirements. 9. Children and service area
This website and MoodClinics’ services are intended for adults age 18 or older. MoodClinics currently provides telepsychiatry to eligible adults who are physically located in Arizona at the time of service.

The site may link to independent services, including a patient portal or referral resources. MoodClinics is not responsible for the privacy practices of independent websites. We may update this policy as services or legal requirements change. The effective date above identifies the current version.
