A clinical relationship begins only after MoodClinics confirms acceptance and applicable intake, consent, and appointment requirements are completed.
5. Eligibility and telehealth
Services are currently intended for adults age 18 or older who are physically located in Arizona during appointments. Identity, location, technology, clinical appropriateness, and other eligibility requirements may be verified. Telehealth may not be appropriate for every condition or circumstance.
6. Appointments, fees, and treatment decisions
The website currently describes a self-pay fee of $99 per visit. Current pricing, availability, cancellation rules, and payment requirements will be confirmed before service and may change. Treatment and prescribing decisions are based on independent clinical judgment. No medication, controlled substance, diagnosis, letter, form, or specific outcome is promised or guaranteed.
